At Shalom Lifecare Solutions LLC, our Developmental Disability In-Home Service is designed to support individuals in their homes through structured care and daily guidance. This service helps promote greater participation in routines through skill-building, personal assistance, and behavioral support. Care plans follow the Individual Support Plan (ISP) and are delivered with consistency, coordination, and a focus on practical progress. Our team works collaboratively with families and coordinators to ensure services align with identified needs and goals. Each aspect of care is handled with professionalism and a clear focus on encouraging increased independence.

Our services include, but are not limited to:

1. Personal Assistance Services 

  • Support with Activities of Daily Living (ADLs):
    Bathing, dressing, grooming, eating, toileting, mobility, and transfers.
  • Medication Assistance:
    Delivered within the provider’s scope, following state guidelines.
  • Health Monitoring:
    Support includes reminders and basic checks like vital signs.
  • Meal Preparation & Feeding Support:
    Services are aligned with any special dietary needs.
  • Household Tasks:
    Includes light cleaning, laundry, and organization to maintain a healthy environment.

2. Skill Building and Training (Based on ISP)

  • Self-Help Development:
    Training in hygiene, dressing, and personal routines.
  • Money Management:
    Practical support for budgeting and basic purchasing tasks.
  • Routine Management:
    Helps with organizing tasks, awareness of time, and maintaining schedules.
  • Travel & Mobility Training:
    Community navigation and basic public transportation skills.
  • Personal Safety Education:
    Emergency responses, home awareness, and fire safety are addressed.

3. Behavioral Support Implementation

  • Behavior Plan Implementation:
    Staff follow approved strategies to support positive habits.
  • Data Reporting:
    Behavioral observations are documented and shared with the appropriate professionals.
  • Crisis Management:
    Trained staff respond to escalations using calm, structured techniques.
  • Reinforcement of Positive Behavior:
    Encouragement of appropriate actions in home and community settings.

4. Socialization and Community Engagement (As Authorized)

  • Communication Support:
    Assistance with AAC devices and expressive communication.
  • Community Involvement:
    Includes support for events, volunteer work, and social participation.
  • Peer Interaction:
    Help with initiating and maintaining relationships.
  • Support for Learning & Work Goals:
    Assistance with educational or vocational steps (if part of ISP).

5. Health Maintenance Activities (As Guided by DMAS)

  • Healthcare Follow-Up:
    Help with scheduling or coordinating medical visits.
  • Medical Visit Support:
    Staff can accompany individuals if this is included in the ISP.
  • Assistive Equipment Help:
    Guidance on using medical or mobility devices correctly.

6. Family / Caregiver Training and Support

  • Caregiver Coaching:
    Instruction based on ISP strategies.
  • Respite Care:
    Short-term relief for caregivers (if authorized in the ISP).
  • Ongoing Updates:
    Open communication regarding progress and needs.
  • Access to Planning Resources:
    Supportive materials and tools for family caregivers.

Service Coordination Activities

  • Collaboration with Support Coordinators:
    Consistent participation in ISP meetings and plan reviews.
  • Documentation Responsibilities:
